Our work began with caregiving support for employees and their loved ones—now we’re extending that care to individuals on Medicare and the people who support them. We’re blending clinical expertise with whole-person care to help people manage health and social needs with less stress and greater confidence.

As a Care Navigator, you’ll be part of a growing team that’s changing the way people experience care—providing telehealth-based support that bridges the gap between healthcare and everyday life. You’ll guide Medicare beneficiaries and their families through challenges like managing chronic and complex conditions, accessing community resources, and navigating mental health—all while working remotely with the flexibility and autonomy of an independent contractor.
How You’ll Make an Impact:
Establish meaningful relationships with members and their families, grounded in empathy & trust
Perform assessments and develop personalized care plans that address medical, emotional, and social needs
Coordinate care across providers and systems, helping members receive the right care at the right time—whether that’s accessing community resources, navigating benefits, or supporting members as they navigate life with chronic conditions
Document all interactions and recommendations with clarity and timeliness, supporting continuity of care and team collaboration

Partner with Grayce’s interdisciplinary team of clinicians and care professionals to continually refine and strengthen our care model
What You Bring:
Active LCSW license in good standing
Specialization in care coordination, case management, or clinical advocacy—preferably serving Medicare or older adult populations. preference for individuals with demonstrated experience navigating care for individuals with complex and acute medical and behavioral health conditions
A deep understanding of Medicare, social drivers of health, and complex care systems
Exceptional communication and interpersonal skills, with a passion for serving aging adults and their caregivers
Experience and comfort with using technology and telehealth platforms, and a readiness to adapt in a fast-evolving care environment
